I'm a little confused as to the placement of a miller type top feeder. Do I place the feeder directly over the brood box with the inner cover on top of the feeder or do I place the feeder on top of the inner cover?

You place the feeder directly on top of the brood chamber, no need for an inner cover the bee's will not be able to access it.

Marc, As a side note; make sure that you don't use shims in between your brood chamber and feeder, the bee's will bridge the gap-literally. I know a guy who did that.
